Job Summary:
We are seeking a qualified Occupational Therapist to join our home health team. The role involves providing patient-centered care across assigned territories, with flexible options for guaranteed or non-guaranteed hours, including travel outside the area if necessary.

Responsibilities:

• Assess and develop personalized treatment plans for patients in their homes.
• Administer therapeutic interventions to improve patients' functional abilities.
• Complete required documentation, including OASIS assessments and progress reports.
• Collaborate with patients, families, and healthcare teams to ensure quality care.
• Travel within designated areas, potentially outside the region, including weekends if required.
• Adhere to all documentation, safety, and compliance standards, including COVID-related protocols.

Qualifications:

• Hold current CA Occupational Therapist license.
• Possess BLS certification and valid driver's license with auto insurance.
• Demonstrate compliance with required immunizations and medical clearances (TB, MMR, Varicella, HepB, TDap, Flu).
• Be flexible to travel and work weekends, if needed.
• Ability to provide care to COVID-positive and PUI patients.
• Have reliable transportation and a clean driving record.
