Job Summary:
Provides pharmaceutical care through patient-centered clinical services, medication management, and education. Ensures the safe and effective use of medications by applying clinical judgment and collaborating with healthcare teams to optimize patient outcomes.

Responsibilities:

• Dispense medications accurately and efficiently while adhering to all regulatory guidelines.
• Conduct comprehensive patient assessments, monitor therapy, and adjust medication plans as needed.
• Educate patients and their families on medication use, potential side effects, and safety precautions.
• Collaborate with healthcare providers to develop and implement individualized care plans.
• Maintain thorough documentation of pharmacy activities using Epic or similar electronic health record systems.
• Ensure compliance with privacy policies, safety standards, and all applicable laws.

Qualifications:

• Hold a valid PHARMD license or registered pharmacist credential, with equivalent experience accepted.
• Graduate from an accredited pharmacy school, Doctorate preferred.
• Possess at least 2 years of pharmacy experience.
• Have valid state licensure and be available for required lab work within 3–5 days.
• Demonstrate proficiency in clinical charting, with Epic experience preferred.
• Maintain current AHA certifications and a clean background, with no DUIs or background hits.
