Job Summary:
Provide professional physical therapy services to patients to improve mobility, reduce pain, and enhance quality of life. Collaborate with healthcare teams to develop individualized treatment plans and ensure optimal patient outcomes.

Responsibilities:

• Assess patients' physical conditions and establish appropriate treatment goals.
• Implement therapy plans utilizing evidence-based techniques.
• Document patient progress and update treatment plans accordingly.
• Educate patients and their families on exercises and injury prevention strategies.
• Maintain compliance with regulatory and facility policies.

Qualifications:

• Hold an active state license as a Physical Therapist.
• Possess a degree from an accredited physical therapy program.
• Demonstrate strong clinical assessment and intervention skills.
• Effectively communicate with patients and healthcare team members.
• Prior experience in a similar outpatient or skilled nursing setting preferred.
