Job Summary:
Provide specialized physical therapy services to patients in a skilled nursing and rehabilitation setting, assisting them in achieving optimal functional outcomes and recovery.

Responsibilities:

• Develop and implement individualized treatment plans based on patient assessments.
• Monitor and document patient progress, adjusting therapy plans as needed.
• Collaborate with healthcare team members to coordinate patient care.
• Educate patients and families on therapeutic exercises and rehabilitation strategies.
• Ensure compliance with regulatory and safety standards during therapy sessions.

Qualifications:

• Current license or certification as a Physical Therapist.
• Proven experience in a skilled nursing or rehabilitation environment preferred.
• Strong knowledge of therapeutic techniques and patient care standards.
• Excellent communication and interpersonal skills.
• Ability to work flexible hours, including weekends on rotation.
