Seeking a dedicated Physical Therapist to provide comprehensive rehabilitative services in an assisted living environment. The role involves evaluating patients, developing treatment plans, and facilitating recovery to enhance residents' quality of life.

Responsibilities:

• Evaluate residents’ physical condition and establish individualized therapy plans.
• Implement therapeutic interventions aligned with best practices and patient needs.
• Document patient progress and adjust treatment plans accordingly.
• Collaborate with healthcare team to coordinate care and optimize outcomes.
• Educate residents and caregivers on exercises and injury prevention strategies.

Qualifications:

• Hold a valid state Physical Therapist license.
• Possess prior experience in a long-term care or assisted living setting.
• Demonstrate proficiency in developing and executing treatment plans.
• Show excellent communication and interpersonal skills.
• Maintain current CPR and clinical certifications as required.
