Job Summary:
Provide skilled physical therapy services to patients in a skilled nursing facility and assisted living setting. Assess, develop, and implement treatment plans to promote patient recovery and functional independence.

Responsibilities:

• Evaluate patient's physical condition and develop individualized treatment plans.
• Administer therapeutic techniques to improve mobility, strength, and coordination.
• Document patient progress and adjust treatment plans accordingly.
• Collaborate with interdisciplinary team to optimize patient care.
• Educate patients and families on exercises and injury prevention.
• Maintain compliance with regulatory and facility policies.

Qualifications:

• Hold a valid Physical Therapist license.
• Possess previous experience in SNF and ALF environments preferred.
• Demonstrate excellent assessment and communication skills.
• Ability to work independently within a multidisciplinary team.
• Be CPR certified.
