Job Summary:
Seeking a dedicated Physical Therapist to provide comprehensive rehab services within a vibrant senior living community. The role involves delivering patient-centered care across multiple settings, including long-term care, assisted living, and independent living environments.

Responsibilities:

• Evaluate and develop individualized treatment plans for residents to improve mobility and function.
• Administer therapeutic interventions aligned with best practices and patient needs.
• Collaborate with multidisciplinary teams, including rehab services and activity staff, to ensure holistic care.
• Document patient progress and modify treatment plans as necessary.
• Educate residents and staff on rehabilitation techniques and injury prevention.
• Participate in community activities and off-site excursions to promote resident engagement and well-being.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• Possess proven experience in geriatric or senior living rehabilitation.
• demonstrate excellent communication and teamwork skills.
• Exhibit a strong commitment to patient care and safety.
• Knowledge of LTC, AL, and ILF facility operations preferred.
• Ability to adapt to a fast-paced, active community environment.
