Job Summary:
Provides skilled home nursing care to patients based on physicians' orders, ensuring quality and compliance with agency policies. Coordinates patient visits, supervises home health aides, and maintains detailed documentation to support ongoing patient care.

Responsibilities:

• Administer personalized nursing care during home visits, following physicians' orders and clinical protocols.
• Supervise and evaluate home health aides and Licensed Practical Nurses/Licensed Vocational Nurses to ensure quality service delivery.
• Document patient assessments, treatments, and progress accurately within the required timeframes.
• Coordinate patient care activities, including wound care, start of care, and follow-up visits, ensuring optimal outcomes.
• Maintain reliable transportation and travel within a designated territory, up to 75 miles from the home clinic.
• Participate in periodic office meetings and maintain proficiency with EMR systems such as HomeCare HomeBase (HCHB).

Qualifications:

• Require a minimum of 1 year of adult home health or hospice experience in a clinical setting.
• Possess a current, active LPN/LVN license in the state of practice.
• Have valid driver’s license, auto insurance, and reliable transportation for home visits.
• Be proficient in HCHB and, for RNs, in Oasis documentation; recent use within 6 months preferred.
• Demonstrate excellent communication skills and ability to work independently.
• Be willing to travel up to 75 miles and accept flexible scheduling, including occasional weekend or on-call duties.
