Job Summary:

Seeking an experienced Registered Nurse to provide comprehensive home health nursing care. The RN will coordinate patient care, ensure compliance with physician orders, and supervise home health aides and LPNs in a fast-paced home care environment.

Responsibilities:

• Assess patient conditions and develop personalized nursing care plans.
• Coordinate and deliver in-home nursing services based on physician orders and agency policies.
• Supervise, evaluate, and mentor Home Health Aides and LPNs to ensure quality care.
• Complete all documentation accurately and timely within the EMR system.
• Travel to patient homes within designated territory, adhering to schedule and mileage requirements.
• Participate in team meetings, training, and ongoing professional development activities.

Qualifications:

• Must have a valid nursing license in the state of practice.
• Minimum of 1 year of recent adult home health or hospice experience.
• Proficient in using HomeCare HomeBase EMR and OASIS documentation.
• Must have reliable transportation, a valid driver’s license, and appropriate auto insurance.
• Ability to travel up to 75 miles from the assigned care center.
• Strong communication, clinical, and supervisory skills.
