Job Summary:
Provides comprehensive nursing care to patients in their homes, coordinating services based on physicians' orders and adhering to agency policies. Responsible for supervising home health aides and LPNs/LVNs while ensuring quality patient outcomes.

Responsibilities:

• Coordinate and deliver nursing care for home health patients in accordance with physician directives and agency protocols.
• Supervise, evaluate, and support home health aides and licensed practical/vocational nurses.
• Document patient assessments, care plans, and daily activities accurately within required timeframes.
• Manage travel between patient homes, ensuring efficient scheduling and patient care continuity.
• Maintain compliance with required certifications, including BLS and CPR, and adhere to all agency and industry standards.
• Participate in office meetings and ongoing training to stay current with policies and clinical best practices.

Qualifications:

• Possess a valid nursing license in good standing in the state of jurisdiction.
• Have at least 1 year of recent adult home health or hospice experience.
• Proficient with HomeCare HomeBase (HCHB) EMR system and, for RNs, OASIS documentation.
• Demonstrate reliable transportation, a valid driver’s license, and adequate auto insurance.
• Strong communication skills with the ability to work independently and supervise staff effectively.
• Ability to travel up to 75 miles from the assigned location and adapt to variable schedules and on-call requirements.
