Job Summary:
Assists patients with rehabilitative therapies under the supervision of a licensed therapist, providing direct care in a home health setting. Executes treatment plans through home visits, ensuring patient progress and compliance with clinical protocols.

Responsibilities:

• Conducts patient assessments and implements prescribed therapy plans during home visits.
• Documents patient progress, treatment activities, and clinical notes accurately and timely.
• Collaborates with the supervising therapist to modify treatment plans as needed.
• Maintains reliable transportation and ensures compliance with agency policies on liability insurance and licensure.
• Travels up to 75 miles from the home clinic to patient residences, accommodating a flexible schedule.
• Participates in periodic office meetings and maintains clinical competence through continued education.

Qualifications:

• Must possess a valid physical therapy assistant license in hand prior to assignment.
• Minimum of 1 year of recent Adult Home Health experience required.
• Must have current CPR and BLS certifications.
• Demonstrates proficiency in home health documentation systems, including HomeCare HomeBase, Oasis, and Outlook.
• Willing to travel up to 75 miles and accept a flexible schedule, including weekend coverage if required.
• Possesses a reliable vehicle, driver’s license, and auto insurance.
