Job Summary:
Seeking a dedicated Physical Therapist Assistant (PTA) for a home health contract assignment. The role involves providing in-home patient care, ensuring therapy objectives are met, and maintaining accurate documentation in a dynamic, patient-focused environment.

Responsibilities:

• Administer prescribed therapeutic treatments and interventions to patients in their homes.
• Document patient progress and therapy activities accurately and within designated timeframes.
• Collaborate with supervising therapists and care teams to develop and adjust care plans.
• Travel to patient homes efficiently, covering designated areas within the required radius.
• Educate patients and caregivers on therapy exercises and home programs.
• Maintain compliance with all licensing, certifications, and home health regulations.

Qualifications:

• Valid license as a Physical Therapist Assistant in the state of practice.
• Minimum of 1 year recent experience in adult home health or hospice therapy.
• Proven ability to perform home visits independently and reliably.
• Strong knowledge of home health documentation systems such as HomeCare HomeBase, Oasis, and PointCare.
• CPR and BLS certifications required.
• Reliable transportation, valid driver’s license, and current auto insurance.
