Job Summary:
Provide comprehensive home health nursing care according to physician orders, adhering to agency policies and protocols. Coordinate patient care, supervise home health aides and licensed practical/vocational nurses, and ensure documentation compliance.

Responsibilities:

• Assess patients in their home environment and develop individualized care plans.
• Administer wound care, medication management, and other nursing procedures.
• Supervise and evaluate the performance of Home Health Aides and LPNs/LVNs.
• Document patient visits accurately within the required timeframe using HCHB EMR.
• Coordinate patient visits, travel, and scheduling to meet productivity expectations.
• Participate in team meetings, on-call rotations, and maintain compliance with all licensing and certification requirements.

Qualifications:

• Hold a valid RN license in the state of practice.
• Minimum of 1 year of recent adult home health or hospice experience.
• Proficient in HomeCare HomeBase (HCHB) EMR system and OASIS (for RNs).
• Reliable transportation, a current driver’s license, and required auto insurance.
• able to travel up to 75 miles within designated territories.
• Demonstrate excellent clinical skills and ability to work independently.
