Job Summary:
Provide comprehensive inpatient case management services for patients through their acute hospital stay. Coordinate discharge planning, transition processes, and ensure utilization management and quality screenings are completed efficiently.

Responsibilities:

• Guide patients through their hospitalization, ensuring smooth clinical processes.
• Oversee discharge planning and facilitate effective transition to post-hospital care.
• Conduct utilization management and perform quality screenings to meet healthcare standards.
• Collaborate with healthcare teams to optimize patient care and resource utilization.
• Maintain accurate documentation of case management activities and patient progress.

Qualifications:

• Possess a valid RN license and BSN degree.
• Have at least 2 years of acute care experience, with strong preference for inpatient case management.
• Demonstrate proficiency in Case Management and Managed Care.
• Hold a CCMC certification or be willing to obtain it.
• Exhibit excellent communication and coordination skills.
