Job Summary:
Provides comprehensive inpatient case management for patients in an acute care setting, coordinating their care from admission through discharge to ensure optimal outcomes and efficient utilization of resources.

Responsibilities:

• Coordinate patient care throughout hospitalization, ensuring smooth transitions and discharge planning.
• Manage all aspects of discharge processes, including patient education and follow-up arrangements.
• Perform utilization management to monitor and evaluate resource use and ensure compliance with standards.
• Conduct quality screenings to maintain high standards of patient care and safety.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Maintain accurate documentation of patient progress and discharge summaries.

Qualifications:

• Possess a valid RN license and a BSN degree.
• Demonstrate at least 2 years of acute care experience, with a strong preference for case management roles.
• Hold a CCM certification or be eligible for it; CCMC preferred.
• Exhibit excellent communication and organizational skills.
• Show ability to perform thorough utilization management and quality assessments.
