Job Summary:
Provide specialized nursing care to residents in a long-term care or skilled nursing facility setting. Ensure quality patient outcomes through compassionate and competent clinical practices.

Responsibilities:

• Assess residents' health conditions and develop personalized care plans.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document patient progress and report changes to the healthcare team.
• Maintain compliance with facility policies and state/federal regulations.
• Educate residents and their families on health conditions and care procedures.
• Adhere to infection control protocols, including COVID-19 vaccination and exemption policies.

Qualifications:

• Valid nursing license in the state of practice.
• Proven experience in LTC or SNF environments preferred.
• Strong knowledge of long-term care regulations and best practices.
• Ability to pass background checks and provide valid identification.
• Demonstrate excellent communication and interpersonal skills.
• Comply with COVID-19 vaccination or exemption documentation requirements.
