Job Summary:
Provides part-time in-home nursing assessments and evaluations for clients enrolled in health programs. Responsible for conducting hybrid telehealth and in-field assessments to determine health status and care needs.

Responsibilities:

• Perform initial comprehensive in-home evaluations, assessing health status, strengths, care needs, and patient preferences.
• Review medical documentation and health referral forms pertinent to each case.
• Enter evaluation data accurately into electronic forms and transmit information as needed.
• Document concerns, conflicting information, and issues identified during assessments.
• Collaborate with consumers to discuss healthcare options, supports, and service vendor choices.
• Communicate effectively between the Call Center and eligible consumers to facilitate enrollment into managed care plans.

Qualifications:

• Current RN licensure in New York State.
• Minimum of 2 years recent experience in a home health setting within the last 15 years.
• Demonstrated ability to conduct comprehensive patient assessments and work independently.
• Proficiency with healthcare technology and electronic data entry systems.
• Strong organizational, communication, and interpersonal skills, with sensitivity to multi-generational populations.
