Job Summary:

Seeking an experienced Registered Nurse specialized in home health care to perform comprehensive patient assessments and coordinate personalized care plans. This role requires a focus on evaluating health status, supporting care continuity, and collaborating with interdisciplinary teams to ensure quality outcomes.

Responsibilities:

• Conduct initial in-home assessments to evaluate patients' health, strengths, and care needs.
• Review medical documentation and referral information relevant to each case.
• Document evaluation findings accurately in electronic systems and transmit data as required.
• Identify and communicate concerns, conflicting information, or issues during assessments.
• Facilitate communication between consumers and the care team to ensure seamless service delivery.
• Support enrollment efforts for managed care health plans and strengthen care coordination.
• Maintain updated knowledge of community resources, payor policies, and network services.
• Contribute to quality review processes by collecting and recording outcome data and case notes.
• Promote health education, prevention, and wellness strategies within the patient community.
• Perform additional duties as assigned by supervisory staff to meet project goals.

Qualifications:

• Valid RN license in New York State is required.
• Minimum of two (2) years recent experience conducting assessments in a home setting.
• Proven ability to work independently in fast-paced, deadline-driven environments.
• Strong interpersonal, organizational, written, and verbal communication skills.
• Familiarity with electronic health records and assessment tools.
• Knowledge of state and agency regulations governing nursing practice.
