Job Summary:

Responsible for providing comprehensive nursing care to residents in a long-term care or skilled nursing facility setting. Collaborates with the healthcare team to ensure residents' well-being, safety, and comfort.

Responsibilities:

• Observe and assess residents’ mental and physical health status continuously and report changes to the supervising nurse.
• Administer medications, treatments, and therapies as prescribed by physicians, documenting all actions accurately in electronic systems.
• Supervise and support Licensed Nursing Assistants (LNAs) in delivering resident care within scope of practice and licensing requirements.
• Assist physicians during resident rounds, addressing medical needs, and facilitating effective communication.
• Complete admissions, discharges, transfers, and detailed 24-hour shift reports, ensuring proper documentation.
• Maintain infection control standards, ensure unit cleanliness, and identify hazards to ensure a safe environment.

Qualifications:

• Hold an active LVN or LPN license issued by the state.
• Minimum of 3 years recent experience in LTC or SNF settings within the last 6 months.
• Demonstrate proficiency in medication administration, wound care, and electronic documentation.
• Willingness to float to different units as needed and obtain IV certification if not already certified.
• Provide proof of current Basic Life Support (BLS) certification through AHA.
• Be fully vaccinated against COVID-19, with proof of vaccination required.
