Job Summary:
The Occupational Therapist provides patient-centered therapy services in a home health setting, focusing on enhancing patients’ functional abilities and independence. The role involves conducting evaluations, developing treatment plans, and implementing therapeutic interventions to facilitate recovery and improve quality of life.

Responsibilities:

• Assess patients' physical, sensory, and cognitive needs through comprehensive evaluations.
• Develop and implement individualized treatment plans aligned with clinical objectives.
• Administer therapeutic activities and exercises to improve patients’ functional status.
• Educate patients and caregivers on strategies, equipment, and activities to support ongoing recovery.
• Document patient progress accurately within EMR systems and ensure timely updates.
• Collaborate with interdisciplinary teams to optimize patient care and treatment outcomes.

Qualifications:

• Hold a valid Occupational Therapist license in the state of practice.
• Minimum of 1 year of recent adult home health therapy experience.
• Demonstrate proficiency in clinical evaluation, treatment planning, and documentation.
• Be CPR and BLS certified.
• Have reliable transportation, a valid driver’s license, and agency-required liability insurance.
• Ability to travel up to 75 miles from the home clinic location.
