Job Summary:
Provides comprehensive home health nursing care to patients following physicians' orders, ensuring quality care and compliance with agency policies. Oversees and evaluates home health aides and licensed practical/vocational nurses to support optimal patient outcomes.

Responsibilities:

• Coordinate and deliver individualized nursing care within patients' homes.
• Document patient assessments, treatments, and progress accurately within required timeframes.
• Supervise and evaluate home health aides and LPNs/LVNs, providing guidance and performance feedback.
• Travel to multiple patient locations within designated territory, maintaining timely schedules.
• Adhere to agency and regulatory standards, including proficiency in EMR systems and OASIS documentation.
• Participate in team meetings and training sessions to ensure ongoing professional development.

Qualifications:

• Hold a valid RN license in the state of practice.
• Minimum of 1 year full-time adult home health or hospice experience.
• Proficient in HomeCare HomeBase (HCHB) EMR system and OASIS.
• Reliable transportation, valid driver’s license, and current auto insurance required.
• Ability to travel up to 75 miles from the assigned care center.
• Strong clinical assessment and documentation skills.
