Job Summary:
The RN Nurse Assessor conducts comprehensive in-home health assessments to evaluate patients' health status, care needs, and preferences. This role involves collaboration with interdisciplinary teams to develop personalized care plans and ensure continuity of care.

Responsibilities:

• Perform initial in-home evaluations of patients, assessing health status, strengths, and care requirements.
• Review healthcare documentation and health referral forms pertinent to each case.
• Input evaluation data into electronic systems and transmit as required for documentation.
• Communicate concerns, conflicting information, or issues identified during assessments.
• Educate consumers on healthcare options, support services, and waiver programs.
• Initiate and facilitate communication between call centers and eligible consumers for enrollment in managed care health plans.

Qualifications:

• Current RN licensure in New York State.
• Minimum of two years of recent experience in a home health setting within the past 15 years.
• Bilingual proficiency in Spanish and English strongly preferred.
• Demonstrated knowledge of professional nursing practices, regulations, and laws.
• Strong organizational, communication, and interpersonal skills.
• Ability to work independently in a fast-paced, deadline-driven environment.
