Job Summary:

Seeking a dedicated Physical Therapist to provide comprehensive home health services in the White River Junction, VT, Lebanon, NH, Bradford, VT, and Windsor, VT areas. The role involves evaluating and treating patients, ensuring personalized care within a cohesive interdisciplinary team.

Responsibilities:

• Assess patients' physical therapy needs and develop individualized treatment plans.
• Implement appropriate therapeutic interventions following professional standards and agency policies.
• Communicate treatment plans and progress with patients, families, and healthcare team members.
• Document evaluations, treatment goals, and patient outcomes accurately.
• Educate patients and their families on therapy techniques and home exercise programs.
• Re-evaluate patient progress and adjust treatment plans as necessary.

Qualifications:

• Hold a BS in Physical Therapy.
• Be a licensed Physical Therapist in VT and NH or possess a current unencumbered compact license.
• Have at least one year of experience working in a home healthcare environment.
• Demonstrate excellent communication skills and the ability to work independently.
• Show commitment to patient-centered care and interdisciplinary collaboration.
