Job Summary:
Provides comprehensive physical therapy services to patients in a home health setting, focusing on optimizing functional independence and safety through skilled interventions. Manages patient care plans, supervises therapy team members, and ensures compliance with federal, state, and organizational standards.

Responsibilities:

• Assess patients through thorough OASIS and PT evaluations, identifying psychosocial, safety, and risk factors.
• Develop and implement individualized care plans with specific goals aimed at improving patient outcomes.
• Deliver skilled physical therapy treatments, including therapeutic techniques and patient/caregiver education.
• Coordinate care with multidisciplinary team members to ensure seamless patient management and communication.
• Supervise and direct clinical activities of PTAs, PT students, and home care aides, ensuring quality standards.
• Complete accurate documentation, manage insurance and billing processes, and address coverage discrepancies proactively.

Qualifications:

• Hold a valid physical therapy license and relevant certifications in accordance with regulatory standards.
• Demonstrate experience in home health or outpatient settings with proficiency in OASIS documentation.
• Possess strong clinical assessment, planning, and treatment skills aligned with APTA guidelines.
• Exhibit excellent communication and interdisciplinary collaboration capabilities.
• Maintain current knowledge of federal, state, and organizational policies affecting patient care.
