Job Summary:
Provide high-quality nursing care to residents in long-term care or skilled nursing facilities as an LVN/LPN. Support patient wellness through clinical expertise and compassionate patient interactions.

Responsibilities:

• Administer medications and treatments to residents following physician’s orders and facility protocols.
• Monitor vital signs, assess patient conditions, and document findings accurately.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Ensure compliance with healthcare regulations and safety standards at all times.
• Educate residents and their families about health management and care procedures.

Qualifications:

• Active LVN or LPN license in the state.
• Proven experience working in long-term care or skilled nursing settings preferred.
• Strong clinical skills in patient assessment and medication administration.
• Ability to follow detailed instructions and maintain accurate documentation.
• Excellent communication and teamwork abilities.
