Job Summary:
Provides comprehensive nursing care to patients in long-term care or skilled nursing facilities, ensuring quality outcomes and patient comfort. Collaborates with interdisciplinary teams to meet individualized care plans.

Responsibilities:

• Assess patient conditions and update care plans accordingly.
• Administer medications and treatments per physician orders.
• Monitor vital signs and observe for changes in patient status.
• Document patient care activities accurately and timely.
• Follow infection control protocols and facility policies.
• Communicate effectively with patients, families, and staff regarding care needs.

Qualifications:

• Valid Registered Nurse (RN) license in the state of practice.
• Proven experience working in LTC or SNF settings preferred.
• Demonstrate strong clinical assessment and critical thinking skills.
• Maintain current CPR and basic life support (BLS) certifications.
• Comply with facility vaccination and PPE requirements.
• Ability to bring a valid ID and adhere to infection control policies.
