Job Summary:
Seeking an experienced Physical Therapy Assistant (PTA) for home health services. The role involves providing client-centered therapy within a home care setting, utilizing Homecare Homebase charting system, and maintaining compliance with state licensure requirements.

Responsibilities:

• Deliver skilled physical therapy interventions to patients in their homes according to physician orders.
• Document clinical notes and maintain accurate records using Homecare Homebase charting system.
• Collaborate with multidisciplinary teams to develop and implement individualized care plans.
• Ensure adherence to home health protocols and travel compliance standards.
• Maintain licensure requirements and clinical competency to support quality patient care.
• Provide occasional weekend coverage and accommodate block scheduling as discussed.

Qualifications:

• Must have at least 6 months of recent or current experience in home health settings.
• Hold an active TN license or a compact license in hand.
• Demonstrate proficiency with Homecare Homebase or comparable charting systems.
• Display strong communication skills and ability to work independently in a home setting.
• Be available for day shifts (8:00 AM – 4:00 PM) and occasional weekend shifts.
• Ability to travel and adapt to different home environments.
