Job Summary:


The RN - Nurse Assessor is responsible for conducting comprehensive health evaluations in both in-home and field settings. This role involves assessing patients' health status, coordinating care plans, and working collaboratively with multidisciplinary teams to ensure optimal patient outcomes.



Responsibilities:

• Perform initial in-home evaluations to assess health status, strengths, care needs, and patient preferences.
  
• Review medical documentation and health referral forms relevant to each case.
  
• Document evaluation findings accurately in electronic systems and communicate findings effectively.
  
• Identify and communicate concerns, conflicting information, or issues encountered during assessments.
  
• Facilitate care continuity by minimizing fragmentation and gaps in treatment plans.
  
• Educate patients about healthcare options, supports, and available community resources.
  
  
  
  Qualifications:
  
• Hold current RN licensure in New York State.
  
• Possess a minimum of two (2) years of recent home healthcare experience within the last fifteen (15) years.
  
• Demonstrate experience conducting assessments for programs such as UAS-NY, OASIS, or similar.
  
• Have experience caring for geriatric populations and managing chronic diseases in a home setting.
  
• Must be proficient with healthcare technology and electronic evaluation tools.
  
• BLS certification required prior to start if not already held.
