Job Summary:
Seeking an experienced Registered Nurse to perform comprehensive in-home assessments for a home health program. The role involves evaluating patient health status, developing personalized care plans, and coordinating with interdisciplinary teams to ensure quality patient care.

Responsibilities:

• Conduct initial in-home evaluations assessing health status, strengths, and care needs.
• Review medical documentation and health referral forms related to the patient.
• Enter evaluation data into electronic systems and transmit as required.
• Document concerns, conflicting information, and issues encountered during assessments.
• Communicate healthcare options and support needs to consumers, including service and waiver options.
• Collaborate with care team members to enhance care coordination and service delivery.

Qualifications:

• Hold current RN licensure in New York State.
• Minimum of two years recent experience in a home health setting within the last fifteen years.
• Demonstrate knowledge of general nursing principles, state laws, and agency regulations.
• Possess excellent organizational, interpersonal, and communication skills.
• Ability to work independently in a fast-paced, deadline-driven environment.
