Job Summary:
Provide skilled physical therapy services to patients in a skilled nursing facility setting. Evaluate, develop, and implement treatment plans to restore and improve patient mobility and function.

Responsibilities:

• Assess patient conditions and develop individualized treatment plans.
• Implement therapeutic interventions to promote healing and mobility.
• Monitor patient progress and adjust treatments as necessary.
• Educate patients and family members on exercises and health management strategies.
• Document patient care activities in accordance with facility policies.
• Collaborate with the healthcare team to coordinate comprehensive care.

Qualifications:

• Active state licensure as a Physical Therapist.
• Proven experience in a SNF or equivalent setting preferred.
• Strong knowledge of rehabilitation techniques and modalities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Commitment to providing quality patient care.
