Job Summary:
Seeking a dedicated Registered Nurse to provide comprehensive nursing care in a Long-Term Care/SNF setting on a contract basis. The nurse will deliver patient-centered care, ensuring safety and compliance with healthcare standards.

Responsibilities:

• Assess residents’ conditions routinely to develop and adjust care plans accordingly.
• Administer medications and treatments adhering to physician orders and facility protocols.
• Document patient care activities accurately in electronic health records.
• Collaborate with multidisciplinary teams to coordinate resident care.
• Educate residents and families on health management and treatment plans.
• Comply with facility policies and safety regulations, including identification and badge requirements.

Qualifications:

• Valid Registered Nurse license in the state of practice.
• Minimum of 1 year of experience in LTC/SNF environments.
• Demonstrate current vaccination against COVID-19 or valid exemption documentation.
• Strong clinical assessment skills and attention to detail.
• Ability to work independently and as part of a team.
• Excellent communication and interpersonal skills.
