Job Summary:

Seeking an experienced Physical Therapist to provide quality rehabilitative care within skilled nursing facilities. The candidate will assess, plan, and implement therapy programs to promote patient recovery and independence.

Responsibilities:

• Evaluate patients' physical conditions and develop customized treatment plans.
• Administer therapeutic interventions such as manual therapy, exercises, and modalities.
• Document patient progress and adjust treatment plans accordingly.
• Collaborate with healthcare teams to coordinate patient care.
• Maintain compliance with regulatory standards and facility policies.

Qualifications:

• Possess at least 1 year of experience in Skilled Nursing Facilities (SNF).
• Have proficiency with NetHealth (Rehab Optima) or similar rehabilitation management software.
• Hold an active California Physical Therapy license.
• Demonstrate strong assessment and treatment skills.
• Possess excellent communication and documentation abilities.
