Job Summary:
The RN Nurse Assessor is responsible for conducting comprehensive pre-evaluation assessments of patients in their homes to develop individualized long-term care plans. The role involves working with healthcare technology, reviewing medical documentation, and collaborating with interdisciplinary teams to ensure seamless patient care.

Responsibilities:

• Perform initial in-home health evaluations assessing patient health status, strengths, needs, and preferences.
• Review patient medical records and health referral forms to gather relevant information.
• Record evaluation data in electronic forms and transmit as required.
• Identify and document concerns, conflicting information, or issues encountered during assessments.
• Communicate with consumers about healthcare options, support services, and waiver programs.
• Facilitate coordination between the Call Center and eligible consumers for managed care enrollment.

Qualifications:

• Hold current RN licensure in New York State.
• Demonstrate bilingual proficiency in Spanish and English (strongly preferred).
• Possess at least two years' recent experience in a home setting within the last 10 years.
• Have experience conducting assessments similar to UAS or OASIS, including working in CHHA, LHCSA, or MLTC.
• Be proficient with healthcare technology and electronic data entry.
• Possess excellent communication, organizational, and interpersonal skills.
