Job Summary:
Provide compassionate and skilled care to adult and geriatric patients within a long-term care and skilled nursing environment. Assist with daily activities, monitor patient needs, and support clinical staff to ensure quality patient care.

Responsibilities:

• Deliver direct patient care by assisting with activities of daily living, including bathing, dressing, and mobility.
• Monitor and record patient vital signs, behaviors, and health status updates.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Follow facility protocols and maintain compliance with state licensure and certification requirements.
• Ensure a safe and clean environment by adhering to infection control and safety policies.
• Float across various units as needed, supporting different patient populations and clinical needs.

Qualifications:

• Hold an active Maryland CNA I certification at the time of submission.
• Possess a minimum of 2 years of GNA experience (CNA experience not substitutable).
• Maintain current BLS certification.
• Demonstrate ability to work every other weekend and adhere to scheduled shifts.
• Must be capable of floating within scope across multiple units, including LTC, LTAC, behavioral health, and rehabilitation.
• Pass a background check and provide references.
