Job Summary:
Provide specialized social work services to patients and their families in hospice settings, supporting emotional, social, and spiritual needs during end-of-life care. Ensure comprehensive patient assessment and coordinate support services to enhance quality of life.

Responsibilities:

• Evaluate patients' psychosocial needs and develop personalized care plans.
• Collaborate with interdisciplinary teams to implement patient-centered care strategies.
• Offer emotional support, counseling, and grief resources to patients and families.
• Manage medication and equipment coordination as part of hospice services.
• Document patient interactions and update care plans according to changing needs.
• Maintain compliance with licensing, safety standards, and agency protocols.

Qualifications:

• Hold an Oregon LMSW or CSWA license; LCSW preferred.
• Possess a valid driver's license with auto liability insurance.
• Maintain current Healthcare Provider BLS certification.
• Demonstrate experience working in a hospice, hospital, or medical setting.
• Show strong communication and interpersonal skills.
• Ability to work independently and manage a personal vehicle.
