Job Summary:
Provide compassionate psychosocial support to patients and families in a hospice setting, ensuring they receive holistic care throughout the end-of-life journey. Collaborate with interdisciplinary teams to address emotional, spiritual, and social needs.

Responsibilities:

• Assess the emotional, social, and spiritual needs of patients and their families through comprehensive evaluations.
• Develop and implement individualized care plans that address psychosocial challenges and support patient comfort.
• Collaborate with healthcare providers, volunteers, and community resources to coordinate supportive services.
• Provide counseling and emotional support to patients and family members facing end-of-life issues.
• Document all assessments, interventions, and care plans accurately within electronic health records.
• Educate patients and families about available resources, community services, and coping strategies.

Qualifications:

• Hold an Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) license.
• Maintain current Healthcare Provider Level BLS certification.
• Possess a valid driver's license and auto insurance, with the ability to use personal vehicle for work-related purposes.
• Have experience working as a social worker in a healthcare or hospital setting preferred.
• Oregon LCSW (Licensed Clinical Social Worker) licensure is preferred.
