Job Summary:

Provides skilled nursing care to patients in a home health setting, coordinating services and supervising health aides and practical nurses. Ensures all care is delivered according to physician orders and agency policies, promoting patient health and safety.

Responsibilities:

• Assess patient needs and develop personalized care plans based on nursing assessments.
• Coordinate and direct home health visits, ensuring timely and appropriate service delivery.
• Supervise and evaluate home health aides and practical nurses to maintain quality standards.
• Document patient progress and care activities accurately within the agency's EMR system.
• Communicate regularly with physicians and the care team to update on patient status and modify care plans as needed.
• Adhere to compliance standards, including proficiency in OASIS and HomeCare HomeBase documentation.

Qualifications:

• Current RN license in the state of practice.
• Minimum of 1 year of recent adult home health or hospice experience.
• Valid driver’s license, reliable transportation, and agency-required liability insurance.
• Proficiency in HomeCare HomeBase EMR and recent usage within 6 months.
• Demonstrated competency in OASIS documentation (for RNs requiring OASIS proficiency).
• Ability to travel up to 75 miles from the assigned care center.
