Job Summary:

Provides comprehensive home health nursing care for clients in their residence, ensuring adherence to physician orders and agency policies through the nursing process. Coordinates patient visits, supervises home health aides and LPNs, and maintains accurate documentation to support quality patient outcomes.

Responsibilities:

• Assess patient conditions and develop individualized care plans based on physician directives.
• Administer skilled nursing interventions, including wound care, medication management, and post-acute care.
• Supervise and evaluate home health aides and LPNs to ensure high standards of patient care.
• Document patient interactions, treatment plans, and progress within the designated EMR system within 24 hours.
• Coordinate and schedule patient visits, travel, and follow-up appointments within assigned territory.
• Participate in team meetings, training, and compliance initiatives to promote best practices.

Qualifications:

• Valid RN license in Oregon with in-hand certification and no restrictions.
• Minimum of 1 year recent adult home health or hospice nursing experience.
• Proficient in HomeCare HomeBase (HCHB) EMR; Oasis proficiency required for RNs.
• Reliable transportation, valid driver’s license, and agency-required insurance.
• Ability to travel up to 75 miles from the assigned clinic and accommodate flexible scheduling.
• Strong clinical assessment skills and the ability to manage a caseload of 5-8 patients daily.
