Job Summary:
Seeking a dedicated Registered Nurse for an LTC/SNF setting to provide comprehensive patient care, ensure regulatory compliance, and support clinical team operations. The ideal candidate will deliver quality nursing services in a fast-paced healthcare environment.

Responsibilities:

• Assess patient conditions and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient progress and document clinical information accurately.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
• Comply with all regulatory requirements, including documentation and safety protocols.
• Educate patients and their families on health management and care plans.

Qualifications:

• Valid state RN license required.
• Proven experience in LTC/SNF environments preferred.
• Must bring a valid ID and wear a photo badge at all times.
• Completion of COVID-19 vaccination or valid exemption documentation required.
• Strong clinical assessment and communication skills.
• Ability to work effectively in a team-oriented setting.
