Job Summary:
Provide high-quality nursing care to residents in a Long-Term Care or Skilled Nursing Facility setting. Support patient well-being through monitoring, administering medications, and collaborating with healthcare teams to deliver comprehensive care.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor patient vital signs and report changes to appropriate medical staff.
• Document patient care activities and observations clearly and promptly.
• Assist with daily living activities, including hygiene, mobility, and nutrition.
• Follow infection control protocols and maintain a safe environment.
• Communicate effectively with residents, families, and team members.

Qualifications:

• Valid LVN or LPN license in good standing.
• Proven experience in long-term care or skilled nursing facilities.
• Ability to monitor and assess patient health status attentively.
• Excellent communication and interpersonal skills.
• Knowledge of medical terminology, medication administration, and safety procedures.
• Ability to work effectively in a fast-paced environment.
