Job Summary:
Seeking a dedicated Physical Therapist to provide patient-centered rehabilitative care across diverse clinical settings. The role involves implementing individualized treatment plans to improve patients' functional ability and quality of life.

Responsibilities:

• Assess patients' physical conditions and formulate appropriate treatment plans.
• Administer skilled therapeutic interventions to enhance mobility and strength.
• Monitor and document patient progress, adjusting treatments as needed.
• Educate patients and their families on therapeutic exercises and wellness strategies.
• Collaborate with multidisciplinary teams to optimize patient outcomes.
• Ensure compliance with all facility and regulatory documentation standards.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• Possess prior experience in outpatient, inpatient, or skilled nursing facilities preferred.
• Demonstrate strong assessment and clinical reasoning skills.
• Maintain current BLS/CPR certification.
• Exhibit excellent communication and interpersonal skills.
• Be available to work scheduled days and shifts as required.
