Job Summary:
Seeking a dedicated Registered Nurse to provide exceptional patient care in a Long-Term Care/SNF setting. The candidate will collaborate with healthcare teams to ensure quality outcomes and compliance with facility protocols.

Responsibilities:

• Assess patient conditions and develop personalized care plans.
• Administer medications and treatments accurately and safely.
• Monitor patient vital signs and respond to changes promptly.
• Document patient progress and maintain detailed medical records.
• Educate patients and families about health management and procedures.

Qualifications:

• Valid RN license in the state of practice.
• Proven experience in LTC/SNF environments preferred.
• Strong knowledge of elderly and chronic disease care.
• Excellent communication and interpersonal skills.
• Ability to work flexible scheduling, including weekends and holidays.
