Job Summary:
A Physical Therapist is responsible for providing rehabilitative services to patients in a home health setting, ensuring optimal recovery and mobility. This role involves conducting assessments, developing treatment plans, and delivering therapy interventions independently in patients' homes.

Responsibilities:

• Evaluate patients' physical conditions and develop personalized treatment plans.
• Administer therapy services including manual therapy, exercises, and modalities.
• Document patient progress and modify treatment plans as needed.
• Educate patients and caregivers on exercises and home programs to promote recovery.
• Collaborate with healthcare team members to ensure continuity of care.
• Maintain compliance with all applicable licensure, insurance, and documentation standards.

Qualifications:

• Hold an active state license as a Physical Therapist.
• Minimum of 1 year of recent home health physical therapy experience.
• Proficient in OASIS assessments and documentation.
• Current CPR and BLS certifications required.
• Must have reliable transportation, driver’s license, and liability insurance.
• Ability to travel up to 75 miles from the assigned clinic location.
