Job Summary:
Provides comprehensive nutritional care to patients and residents with special needs or nutritional risks. Conducts patient assessments, develops nutrition plans, and educates patients and families while collaborating with the healthcare team to ensure optimal nutritional outcomes.

Responsibilities:

• Perform initial and ongoing nutritional assessments to identify patients at risk.
• Develop and implement individualized nutrition care plans based on assessment findings.
• Monitor patient progress and response to interventions, adjusting plans as necessary.
• Educate patients and families on nutrition, dietary choices, and medical nutrition therapy.
• Collaborate with interdisciplinary team members to coordinate nutritional care and communicate patient needs.
• Document all assessments, interventions, and patient responses accurately in medical records.

Qualifications:

• Hold a Bachelor’s Degree in Food and Nutrition or equivalent.
• Possess a Master’s Degree, if eligibility for examination began on or after 01/01/2024.
• Complete a dietetic internship or its equivalent.
• Be a Registered Dietitian (RD) within 6 months of residency program completion.
• Demonstrate at least two years of recent clinical dietitian experience.
• Maintain current BLS certification and valid nursing license as per state requirements.
