Job Description
Job Summary:
The RN Case Manager is responsible for coordinating patient care and facilitating smooth care transitions within an acute care hospital setting. This role involves collaborating with healthcare team members, patients, and families to ensure appropriate utilization of resources and optimal patient outcomes.
Responsibilities:
- Assess initial and ongoing patient needs by reviewing medical records, discharge plans, and conducting patient and family interviews.
- Develop comprehensive, cost-effective care plans tailored to each patient’s clinical and psychosocial needs.
- Collaborate with physicians, social workers, and the interdisciplinary team to coordinate care and facilitate timely patient transitions.
- Monitor and review documentation to ensure compliance with level of care requirements and regulatory standards.
- Identify barriers to care or discharge and work proactively with the team to resolve issues and prevent unnecessary readmissions.
- Oversee post-acute care arrangements, including referrals to community resources and ensuring proper documentation for transfers and discharges.
Qualifications:
- Valid CA RN license and current Nursys verification, uploaded within the last 2 weeks.
- Minimum of 5+ years of hospital case management experience, with inpatient acute care expertise.
- Proven ability to perform care coordination, assessment, and discharge planning within a hospital setting.
- Strong knowledge of CMS, Joint Commission, and regulatory requirements for transitions of care.
- Excellent communication and collaboration skills to work effectively with multidisciplinary teams and diverse patient populations.
- BLS certification, with applicable certifications uploaded and valid.
