Job Description
Job Summary:
The Case Manager RN is responsible for coordinating care and facilitating care transitions for hospitalized patients to ensure optimal health outcomes, effective resource utilization, and smooth patient flow throughout the acute care process.
Responsibilities:
- Assess patient needs upon admission and continuously evaluate care plans to address psychosocial, medical, and financial factors.
- Collaborate with physicians, interdisciplinary teams, and community resources to develop and implement comprehensive, cost-effective care transition plans.
- Monitor patient progress and review documentation to confirm appropriate level of care and expedite discharge planning.
- Review and coordinate post-acute care services, ensuring compliance with regulatory requirements and facilitating smooth patient transfers.
- Identify barriers to care progression, such as social determinants or psychosocial issues, and work with team members to resolve them efficiently.
- Document all assessments, care plans, and interventions accurately within electronic health records and communication platforms.
Qualifications:
- Current CA RN license with Nursys verification within the last 2 weeks.
- Minimum of 5 years of RN experience, including inpatient acute care case management.
- BLS certification required; AHA or ARC certifications preferred.
- Strong knowledge of CMS, Joint Commission, and regulatory Transitions of Care guidelines.
- Proven ability to collaborate with multidisciplinary teams and community agencies.
- Excellent communication, assessment, and critical thinking skills.
