Job Summary:
Provide comprehensive physical therapy services to patients with physical functioning disorders and wound care needs. Develop and implement individualized treatment plans to improve patient outcomes while adhering to regulatory and clinical standards.

Responsibilities:

• Screen and evaluate patients to determine their physical therapy needs.
• Develop and modify personalized care plans based on assessment findings and patient progress.
• Administer therapeutic interventions including manual techniques, ambulation, exercises, modalities, supportive devices, and wound care.
• Assess patient progress and document treatment outcomes accurately in electronic health records.
• Communicate effectively with interdisciplinary team members, patients, and caregivers regarding treatment plans and progress.
• Support supervision of physical therapist assistants, rehabilitation aides, and students as needed.

Qualifications:

• Hold a valid Missouri State License to practice as a Physical Therapist.
• Possess at least 1 year of relevant experience in LTAC or rehab settings.
• Successfully completed Level II clinical experience in physical therapy.
• Demonstrate knowledge of regulatory requirements and documentation standards.
• Exhibit strong communication and interpersonal skills.
