Job Summary:
Provide comprehensive physical therapy services to patients in a skilled nursing facility. Assess, develop, and implement treatment plans to improve patient mobility and function, ensuring optimal care outcomes.

Responsibilities:

• Evaluate patients’ physical condition and develop individualized treatment plans.
• Administer therapeutic interventions to improve mobility, strength, and coordination.
• Monitor patient progress and adjust treatments as necessary.
• Document patient care and progress accurately in compliance with clinical standards.
• Collaborate with interdisciplinary teams to coordinate patient care.

Qualifications:

• Valid state licensure or certification as a Physical Therapist.
• Proven experience working in a skilled nursing or rehabilitation setting.
• Strong assessment and clinical decision-making skills.
• Excellent communication skills for patient interaction and team collaboration.
• Ability to manage a caseload and prioritize treatment schedules effectively.
