Responsibilities:

• Evaluate patients' physical conditions and develop tailored treatment plans.
• Administer therapeutic interventions to improve mobility, strength, and pain management.
• Document patient progress and adjust treatment plans as necessary.
• Collaborate with nursing staff and healthcare professionals to optimize patient outcomes.
• Assist at multiple facilities, including Oakbrook Health and Rehabilitation, as needed.

Qualifications:

• Hold an active state license as a Licensed Physical Therapist.
• Possess experience in skilled nursing or rehabilitation settings.
• Demonstrate strong interpersonal and communication skills.
• Be willing to travel and work across various facilities.
• Ability to adapt treatment plans based on patient needs and progress.
