Job Summary:

The Physical Therapist will provide rehabilitative services to patients in a skilled nursing facility, focusing on improving mobility and quality of life. This role requires collaboration with healthcare teams to develop and implement personalized treatment plans.

Responsibilities:

• Evaluate patients' physical conditions and develop appropriate therapy plans.
• Administer therapeutic exercises and modalities to promote recovery.
• Monitor patient progress and adjust treatments accordingly.
• Document patient assessments, interventions, and outcomes accurately.
• Collaborate with nursing staff and other healthcare professionals to coordinate care.
• Educate patients and their families on post-treatment care and injury prevention.

Qualifications:

• Valid state licensure as a Physical Therapist.
• Proven experience working in SNF or similar long-term care settings.
• Strong knowledge of physical therapy techniques and modalities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Certification in CPR/BLS.
